By checking this box, you acknowledge that you are eligible for local delivery within 25 miles of the Field to Fork shop. You also acknowledge that if someone will not be home to receive your delivery, a cooler must be provided. If no cooler is present, your order will be returned to our shop and made available for pick-up.

By checking this box, you acknowledge that you must pick up your meat box during the time-frame you have specified above on this form. If you miss your pick-up day, any fresh meats will be placed in our freezer and must be picked up within the week.
